Horizon House General Information
IMPORTANT NOTICE HORIZON HOUSE EMPLOYEES Long term nursing care is a serious and costly form of health care. Recognizing this problem, Horizon House offers a new, low cost, long-term care insurance plan to employees and thier families. This plan is underwritten by Unum, one of the nation’s leaders in insurance of this type. It is now available on to all employees who work at least 25 hours per week. Coverage is available on a voluntary, employee paid basis, with Guarantee Issue for new employees. Spouses, siblings, adult children, parents, parents-in-law, grandparents and grandparents-in-law may also apply at Group Rates. Medical evidence of insurability is required.
