How to Sign Up

To enroll for UNUM Long-Term Care Insurance, pickup an enrollment packet at your school or the payroll department. Our enrollers also have a supply available at scheduled meetings. Or call 425-861-8700 and one will be mailed out to you. When done filling out the packet, turn the forms in at your school office or the payroll department and they will be forwarded to Lehmann/Wood & Associates, Inc.
